Tooth 36 underwent perforation repair by Dr. Umeda (@endodontics_tckw). For 37, a distal root fracture initially led us toward implant therapy, but because the patient works in healthcare and was highly cooperative, I proposed Plan B instead. Thank you, Dr. Umeda, as always.

We proceeded with autogenous tooth transplantation. Transplanting the large, curved-root 28 into 37 was challenging, yet the patient’s excellent plaque control allowed uneventful healing—2 mm pockets with no BOP—and we completed the case with a full-zirconia crown.

Bone was harvested under the 37 mesial pontic area with a trephine, ground, mixed 1:1 with graft material, and placed between the root and crestal bone.

Although more demanding than implant therapy, using a viable PDL is invaluable. This case reminded me that success depends heavily on patient selection; the right patient makes all the difference.

今回は、自家歯牙移植に調整。歯牙サイズ大きく、弯曲根の28を37に移植するのはかなりチャレンジケースでした。しかし患者さんの丁寧なプラークコントロールのお陰で、全周ポケット2mmでBOPもなく順調な治癒を辿り、Full Zirconia Crownにて補綴できました。

37近心のポンティック下からトレフィンバーにて採骨し、ボーンミルで挽いて骨補填材と5:5で混合し、歯根と骨頂の間に忍ばせました。

インプラントより難しい移植ですが、やはり使える歯根膜は使いたいものです。しかし、患者さんのレベルによっては上手くいくものも大失敗となりえるのでケースセレクションがとても大切だと痛感。
