The treatment plan for #37 had already progressed to a consultation for extraction and immediate implant placement.
However, the words of the great masters of Perio and Endo—“You can always extract later; preserve the tooth whenever possible”—kept echoing in my mind.

I performed gingivectomy with an electrosurgical unit, removed the caries, built up a resin core, and prepared the tooth vertically. At the next visit, I will fabricate the provisional.
My intention in changing the plan was to keep the tooth functional with its periodontal ligament—whether for two years or five—through a nonsurgical, minimally invasive approach.

I fully understand the value of implants, but I never want to be bound or “cursed” by them. A dentist’s duty is to protect natural teeth and create an oral environment that can be maintained with ease. That’s easy to say, yet long-term preservation is challenging no matter which path you choose.

37はC4で抜歯即時埋入のコンサルまで決まってたんですが、「抜歯はいつでもできる　可及的に歯牙を保存しろ」と言ってたPerio/Endo界の重鎮達の言葉がリフレイン。電メスで歯肉切除し、カリエス除去し、レジンコア築造しVertical preparationしました。次回プロビジョナル作製。非外科的で2年でも5年でも歯根膜使って咬ませてあげられたらいいなと思っての、治療法転換。低侵襲で患者さんを喜ばせられる可能性があれば、そちらを選んで差し上げたい。

インプラントの良さも分かるけど、インプラントに呪われないように。歯科医師は歯を守り、メンテナンスしやすい口腔内をつくるのが職務。言うのは簡単だけど、長期保存はどちらも難しい。
